Requirements can be found at Travel State Passport Application Website.
Here are the list of requirements I prepared:
- Complete Form DS-11 for first time applicant
- Naturalization Certificate ( as evidence of Citizenship )
- Photocopy of Naturalization Certificate – in my experience, they photocopied my original Naturalization Document.
- ID – I showed California Driver’s License
- Original Birth Certificate
- Fees in a separate check:
Application Fee: $110
Execution Fee: $35 - US Passport Photo ( I’ve done ours at Costco)

Received my Passport after 3 weeks!

Hi. I have been following your blog for a long time now. It’s been very informative and helpful for me as I have a daughter as my K2 dependent too. I am just wondering, did your son got his US passport the same time you did? Or did they require him a certificate? Thank you so much:)

Hi Shirley, Thanks for stopping by. My son and I applied for US Passport at the same time, however we need to provide more information to state department in order for his US Passport to get approved. See link https://bit.ly/2BlUyR8
